This page is to better serve our clients and save time when checking in. Attached you will find the necessary forms for your visit to our hospital. It is very important that you take the time to read and complete the entire form(s). If this will be your first visit to Lineberger Veterinary Hospital, we welcome you and ask that you complete the "Client Information Form". Please make sure to bring along all previous records for your pet. This will ensure that we have a history on the patient and that all vaccinations are current.

When printing consent forms, please be sure to print all forms pertaining to your pet's visit. For example: If your dog is having a growth removed but is also scheduled for a teeth cleaning, please fill out the "Growth Removal Consent Form" and the "Dental Consent Form". If you have any questions about which form(s) are needed for your appointment please call (704-824-8451) and our receptionists will be glad to assist you.
